How to Fill the Form
Fill the form in black ink and capital letters. All fields marked with an asterisk are mandatory. You will need to enter the date, your customer
ID, full name as printed on the Aadhaar card, your Aadhaar number, and your signature at the bottom. Do not use correction fluid — if you make a mistake, print a fresh copy.
